Parental Consent/Waiver

The safety of your child is our primary concern. Precautions will be taken for their well-being and protection.
The Church will make every reasonable effort to ascertain that, whether at the church or at an off-site activity:
(a) the supervisors and staff of the Service Provider are fully trained and qualified.
(b) the children who undertake the program or activities will be adequately supervised
(c) the location and/or facilities meet the applicable health and safety standards
(d) any equipment made available by the Service Provider or used in the activity has been inspected and is deemed to be appropriate, safe and well maintained.
(e) the location where the activity will take place is appropriate and safe

1. I am satisfied that I have been informed of my right to obtain as much information about this program or activity as I feel necessary, including information beyond that information provided to me by the Church to the extent I require and am not, in any way, relying solely upon information provided by the Church respecting the nature and extent of the risks and hazards associated with the program or activity.

2. I freely and voluntarily assume the risks and hazards inherent in the nature of the program or activity, and understand and ACKNOWLEDGE that my child, as a participant, may suffer personal injury due to any unforeseeable events. This includes anything related to COVID 19.

3. I/we, named above, undertake and agree to indemnify and hold blameless the staff and volunteers of Varsity Bible Church from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Varsity Bible Church, as well as of any medical treatment authorized by the supervising individuals representing the church. This consent and authorization is effective only when participating in or traveling to events of the Varsity Bible Church.

4. My child has been informed that he/she is to abide by the rules and regulations including directions and instructions from the Church's administrators, instructors and supervisors as imposed on children while participating in the program or activities. This shall include his/her participation in all of the introductory sessions and meet all prerequisites prior to his/her participation in the program or activity, if any.

5. I acknowledge that it is my responsibility to advise the Church of any medical or health concerns of my child which may affect his/her participation in the stated program or activity. This information is confidential. If necessary, I authorize the leaders to administer first aid in the event of an accident, injury or sickness if necessary, until more qualified help can be obtained.

6. Based on my understanding, acknowledgement and consents as described herein, I agree that this child has my permission to participate in the children's ministry activities within the church building, on the property surrounding the church for the program year effective from date signed for one year.

Purpose and Extent

Varsity Bible Church is collecting and retaining this personal information for the purpose of enrolling your child in our programs, to assign the student to the appropriate classes, to develop and nurture ongoing relationships with you and your child, and to inform you of program updates and upcoming opportunities at our Church. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel.
