All information will be kept confidential by church leadership and staff and will not be disclosed unless required by law. Answering "Yes" to any of the questions may not necessarily prevent you from volunteering with the church, however we will arrange for a follow-up conversation.

Statement of Faith

I do believe and adhere to the following Statement of Faith *

1. We believe in the sovereignty, love, and grace of God the Father, the Son and Holy Spirit in creation, providence, revelation, redemption and final judgement.
2. There is one God, eternally existent in three Persons: Father, Son, and Holy Spirit.
3. We believe in the divine inspiration of Holy Scripture and its entire trustworthiness and supreme authority in all matters of faith and conduct.
4. Our Lord Jesus Christ is God manifest in the flesh. We affirm His virgin birth, sinless humanity, divine miracles, bodily resurrection, ascension, ongoing mediatorial work, and personal return in power and glory.
5. The salvation of lost and sinful humanity is possible only through the merits of the shed blood of the Lord Jesus Christ, received by faith apart from works, and in characterized by regeneration by the Holy Spirit.
6. The Holy Spirit enables believers to live a holy life, to witness and to work for the Lord Jesus Christ.
7. The Church, the Body of Christ, consists of all true believers.
8. We believe in the expectation of the personal, visible return of the Lord Jesus Christ, our participation in the resurrection, and the hope of eternal life.

Release of Information and Declaration of Intent *

I hereby give Varsity Bible Church (the "Church") consent to verify the information provided by me in this Volunteer Application Form and to contact the references listed above and to obtain and verify any information from them (and any other persons that the Church determines might be able to provide relevant information) that may be relevant to my application.
I grant my permission for the Church to perform a police records check on me.
I understand that if the Church approves my volunteer application and later determines, in its discretion, at any time that I am not suitable for volunteer service in the Church or for the volunteer position for which I am applying, the Church may terminate my volunteer service or volunteer position for any reason without advance notice.
If the Church approves my application for a volunteer position, I will sign any documents that the Church requires and will at all times cooperate fully with the staff of the Church in the fulfillment of my duties and will keep all confidential information I encounter in my role as a volunteer, confidential.
If at any time I determine that for any reason I am unable to support or adhere to or follow the policies, procedures or doctrine of the church, I will inform the Church and will resign my volunteer position.
I hereby acknowledge that, to the best of my knowledge, the information contained in this Volunteer Application Form is true and correct.
